The application will ask for some basic information, along with a few other things to help us understand your strengths:

1. A current resume or CV.

2. Transcripts (unofficial is OK) of your college-level coursework.

3. Contact information for someone, ideally a professor, that can comment on your background and your readiness for an introductory research experience, in a reference letter.
